us/tanelle.jpgDobson and Associates offers telephonic and on-site Case Management services to assist with the care and management of the more complex and catastrophic medical conditions that are typically responsible for the greatest cost to employers.

These more seriously ill patients have multiple physicians and/or providers, medications, services, co-morbidities and diagnoses that often require extra care and attention to manage their health and promote the effective use of available resources to maximize their health improvement.

Our case managers are highly skilled professionals with a wide array of experience in managing the complex issues associated with catastrophic illnesses, injuries and chronic illness.

Our case managers include registered nurses that are certified case managers by the Commission for Case Management Certification registered nurses with advanced degrees and social workers. They are liaisons between patients, family members, physicians, specialists, insurance companies, employers and other health care professionals to promote the delivery of high-quality care at the most cost-efficient setting based on their individual care needs.

 


 The typical cases referred for Case Management evaluation include:

 

  • High claims (over $35,000.00)
  • HIV/AIDS
  • Spincal Cord Injuries
  • Head/brain injuries
  • Multiple system failure
  • Long-term intravenous therapy (TPN, antibiotics, etc.)
  • Ventilator/trach dependency
  • High Risk Pregnancy
  • Organ transplants
  • Cystic Fibrosis
  • End stage organ disease (liver, kidney, etc.)
  • High risk or ill newborns (birth defects, premature delivery, etc.)
  • Cancer
  • Stroke
  • Neuromuscular disorders
  • Alcohol and/or substance abuse
  • Burns
  • Chronic respiratory/lung disorders
  • Long-term enteral therapy
  • Frequent hospitalizations

Our Case Management teams consist of highly trained, experienced registered nurses and licensed clinical social workers, each with a minimum of 5 years Case Management experience prior to joining the team. The role of the Case Manager’s is to assist those members who are catastrophically ill or injured negotiate the health care system and their benefit options. We offer a comprehensive program to all clients that encompasses both medical and psychosocial issues impacting the member’s recovery. We strive to ensure the member receives the appropriate care in the most appropriate and cost-effective setting while empowering the member to monitor and track events that are important to managing and maintaining health care costs.
